• Diagnosis code
• Dosing instructions (total dose, quantity, frequency, and refills)
• Copy of patient insurance card (front and back)
• Patient height, weight, and date of birth
• Patient / caregiver contact information (phone, mobile phone, email)
• Patient medical records supporting treatment (laboratory reports, treatment history, etc.)
